Medical Release
We have partnered with MediCopy to fulfill your Release of Information requests. MediCopy is fully HIPAA compliant and adheres to all state and federal regulations concerning release of medical information. To request your medical records visit medicopy.net/axia.
Patients can request copies of their medical records at no cost, but delivery fees apply for postage! Please provide an email address on the authorization to expedite the process and delivery method. Records sent via email or to another provider are complimentary.

IMPORTANT NOTE
If this is your first mammogram at Axia Women’s Health and you’ve had prior mammograms elsewhere, please try to obtain images and bring them with you to your appointment so the previous images can be compared to the new ones. You can request records be sent to Axia by filling out a release form which will be sent to you through text upon scheduling your appointment or you can complete a release when you arrive for your appointment.
